Appointment Times

    Monday - 9 a.m. to 8 p.m.
    Tuesday - 10 a.m. to 5 p.m.
    Wednesday - 9a.m. to 8 p.m.
    Thursday - 9 a.m. to 5 p.m.
    Friday - 10 a.m. to 8 p.m.
   

Clinic hours vary with demand and are subject to change.  Dr. Choi will usually continue working until everyone is seen on Modays, Wednesdays, and alternate Fridays, subject to other committments.  Existing patients can always ask for same day appointments.

Clinic Hours

The clinic is usually staffed 8 a.m. to 8 p.m., Mondays Wednesdays and Fridays; and to 5 p.m. on Tuesdays and Thursdays.  We may close early on days when Dr. Choi is not working late.

To Make An Appointment

    1) call the clinic at 206-453-4215 during office hours, or

    2) come into the clinic, relax, enjoy a beverage, and choose your appointment time, or

    3) email requested appointment time and other details to appt@hometownedoc.com, and we will respond within 24 working hours

New Patients

New patients are welcome at Home Towne Family Medicine. Please arrive 15 minutes prior to your appointment for registration if you have not seen us before. Many of the pre-registration froms can be found on our HTFM Patient portal. If you provided your email during registration of your first appointment you can Sign In . You can request your prior medical records to be sent to us prior to your appointment to make the first visit more comprehensive. Here is a medical record transfer consent form that you can electronically fill out, sign, and submit to us. We will handle the rest.

Emergencies

In the event of an emergency, please call 911. It is always safer to go to the hospital via 911/ambulance/AIDE car if you have symptoms such as abdominal pain, chest pain, or major trauma. Do not wait to first talk to the doctor or nurse at this clinic if you believe you have an emergency.

After hours

Please call back during business hours to make or change appointments, or if your question can wait for the next business day. Alternatively, you can leave a message and the staff will get back to you the next business day. If you must talk to medical personnel, you can call Sound Consulting Nurse Service at 206-805-6228 after office hours. They have protocols to answer most medical questions, or can refer you to the doctor on call.

On Call Doctor

We participate in the South Call Group. When you call after hours, the consulting nurse will contact the on-call doctor as needed. The phone number for the consulting nurse after hours is 206-805-6228.
