After hours : Please call back during business hours to make or change appointments, or if your question can wait for the next business day.  Alternatively, you can leave a message and the staff will get back to you the next business day.  You can also call Sound Consulting Nurse Service at 206-805-6228 after office hours.  They have protocols to answer most medical questions, or can refer you to the doctor on call.

Emergencies : In the event of an emergency, please call 911.

On Call Doctor : We participate in the South Call Group.  Consulting nurse will contact the on-call doctor as needed.

Insurances : We currently accept most major insurance plans.  We do accept HMO plans by our HMSO, such as AARP Medicare Complete by United Health Care / Secure Horizons.  We do not accept Medicaid or DSHS as primary or secondary insurance at this time.

Billing : Co-pays are due before you see the doctor.  Please direct all billing questions to the director or manager.

Appointment Times :

The following are appointment times for the clinic.  Clinic hours may vary with demand.  Dr. Choi will usually continue working until everyone is seen, except when in case of other unalterable committments.  Existing patients can always ask for same day appointments.

Dr. Choi:

    Monday - 9 a.m. to 8 p.m.
    Tuesday - 1 p.m. to 5 p.m.
    Wednesday - 9a.m. to 8 p.m.
    Thursday - 9 a.m. to 5 p.m.
    Friday - 1 p.m. to 8 p.m.
   
Office Hours :

The clinic will be staffed 8 a.m. to 8 p.m., Monday to Friday.  There may be early closure on days when Dr. Choi is not working late.

To Make An Appointment :

    1) call the clinic at 206-453-4215 during office hours, or

    2) come into the clinic, relax, enjoy a beverage, and choose your appointment time, or

    3) email requested appointment time and other details to appt@hometownedoc.com, and we will respond within 24 working hours





New Patients :
New patients are welcome in the clinic.  A patient should come in 15 minutes prior to the appointment for registration if they have not been seen by Dr. Choi before.  You can request your prior medical records to be sent to us prior to being seen to make the first visit more comprehensive.  Here is a medical record transfer consent form that you can print, fill out, sign, and fax to us.  If you are a former Dr. Griffith patient, you can sign a simpler form when you arrive at the clinic. 

Prescription Refills : Please contact your pharmacy to initiate prescription refills 3 days before you run out. Sometimes medicines can only be refilled after an appointment or additional lab tests.   Controlled substances, such as narcotic pain killers, can only be refilled during an appointment. 

Physicals and Periodic Health Reviews : Dr. Choi prefers seeing you with all labs already completed, so that periodic health planning can be done in one session.  Please contact the clinic for labs that need to be done prior to your appointment.  Please note that not all insurance plans cover 'physicals' or labs associated with them, even if they are generally considered 'normal' or beneficial.  Please check with your insurance carrier as to what specific items they will cover.
